特定左颞叶与内侧颞叶结构的癫痫样棘波破坏言语情景记忆编码

摘要
诊断为癫痫的患者经历认知功能障碍,这可能是由于自发性癫痫样棘波引起的短暂性认知/记忆损害(TCI/TMI)。我们在166名药物难治性局灶性癫痫成年患者队列中探究,在言语情景记忆编码期间特定神经解剖区域的棘波是否会显著降低回忆概率。我们使用朴素贝叶斯机器学习模型发现,当棘波出现于左布罗德曼21区(BA21)时正确单词回忆概率显著降低11.9%(p<0.001),左BA38降低49.7%(p=0.01),右BA38降低32.2%(p<0.001),左BA36降低21.4%(p<0.01)。我们还考察了癫痫起始区与语言优势半球对该效应的影响。我们的结果表明,自发性癫痫样棘波在已知对语义加工和情景记忆重要的脑区产生显著的TCI/TMI效应。因此,癫痫患者的记忆损害可能归因于与异常发作间期电事件相关的细胞事件。
