乳腺密度:多厂商数据集上视觉评估与全自动计算的比较

摘要
目的:在多厂商数据集上,比较自动化乳腺密度评估器(ABDE)所提供的乳腺密度(BD)评估与一组经验丰富的乳腺放射科医生所提供的评估。方法:21名放射科医生使用BI-RADS a、b、c和d密度分类,对来自9个中心和6个不同厂商的613张筛查/诊断数字乳腺X线摄影图像进行了评估。相同的乳腺X线摄影图像也由ABDE进行评估,该评估器提供纤维腺体与全乳腺面积的连续比值,并自动给出BI-RADS评分。以小组多数报告(PMR)作为参考标准。针对二分类(BI-RADS a-b versus c-d)和4类分类计算了一致性(k)和准确度(正确分类的案例比例)。结果:虽然个别放射科医生与PMR的一致性介于k=0.483至k=0.885之间,ABDE正确分类了613张中的563张乳腺X线摄影图像(92%)。在二分类中,个别读者之间(k=0.620,标准差[SD]=0.140)、个别与PMR之间(k=0.736,SD=0.117)以及个别与ABDE之间(k=0.674,SD=0.095)均发现实质性一致。ABDE与PMR之间几乎完全一致(k=0.831)。结论:ABDE在多厂商数据集的二分类BD评估中与21名放射医生小组几乎完全一致,有望成为视觉评估的可重复替代方案。
