Inter-Electrode Pulse Wave Velocity: 用于妊娠期母亲动脉硬度评估的多通道ECG方法

目标:验证一种 novel 的电极间脉冲波速度(PWV)方法,该方法在无需左心室射血时间(LVET)估计的情况下测量ECG电极之间的脉冲传播。方法:我们分析了来自FELICITy 2队列(约19和35周孕周)的43例三通道ECG记录(1000 Hz)。独立地通过集成方法检测每个通道的R峰。电极对之间匹配R峰的时间滞后(Delta t)用于计算PWV,公式为PWV = L / Delta t,其中L为有效电极间距离。三个通道对提供独立的PWV估计。通过滑动窗口(1-15分钟)评估了时间稳定性。为测试Delta t是反映形态学还是血管传播,我们评估了三个QRS fiducial点(R峰、QRS起始、最大dV/dt)和两个带通滤波器(0.5-40 Hz和0.5-100 Hz)。控制组(n=24)和瑜伽组(n=20)的纵向变化进行比较。结果:PWV值在生理上合理,与主动脉PWV(5-10 m/s)一致:控制组7.40 ± 1.51 vs 6.98 ± 1.63 m/s;瑜伽组7.10 ± 2.15 vs 8.16 ± 0.91 m/s(早期 vs 晚期妊娠)。PWV在5分钟时稳定(变异系数12.3%),比心率和心率变异性低2.6至5.2倍。电极间延迟(15-27 ms)在不同fiducial点和滤波设置下持续存在,表明其不受纯形态畸变影响;PWV维持在6.8-9.1 m/s范围内。初步组间趋势不同(控制组-5.7%,瑜伽组+14.9%;互作用p=0.07)。结论:电极间PWV enables direct spatial assessment of pulse propagation with physiologically valid values, is robust to fiducial and filtering choices, and shows promise for pregnancy-related arterial stiffness assessment with standard multi-channel ECG。
