在 BRATS 挑战中识别用于脑肿瘤分割、进展评估与总生存期预测的最佳机器学习算法

摘要

胶质瘤是最常见的原发性脑恶性肿瘤,具有不同程度的侵袭性、可变预后及多种异质性组织学亚区,即瘤周水肿/浸润组织、坏死核心、活动与非增强核心。这种内在异质性也描绘于其放射表型中,因其亚区由多参数磁共振成像(mpMRI)扫描中散布的不同强度分布刻画,反映不同的生物学特性。其异质性形状、范围与位置是使这些肿瘤难以切除、某些情况下无法手术的因素。在评估表观肿瘤以潜在诊断进展时,切除肿瘤的量也是纵向扫描中考虑的因素。此外,越来越多证据表明,各肿瘤亚区的准确分割可为面向患者总生存期预测的定量图像分析提供基础。本研究评估了过去七届国际脑肿瘤分割(BraTS)挑战赛(即 2012–2018)中用于 mpMRI 扫描脑肿瘤图像分析的最先进机器学习(ML)方法。具体而言,我们聚焦于:i) 评估术前 mpMRI 扫描中各类胶质瘤亚区的分割;ii) 借助肿瘤亚区纵向生长评估潜在肿瘤进展,超越 RECIST/RANO 标准的使用;iii) 从接受全切除患者的术前 mpMRI 扫描预测总生存期。最后,我们探究识别各项任务最佳 ML 算法的挑战,考虑到除各届挑战赛本身多样外,多机构 mpMRI BraTS 数据集亦为持续演化/增长的数据集。
