重度抑郁症(MDD)是一种复杂的精神障碍,影响全球数亿人的生活。即便至今日,研究者仍争论脑形态学改变是否与 MDD 相关,这很可能源于该疾病的异质性。将能够捕捉复杂非线性模式的深度学习工具应用于神经影像数据,有潜力为 MDD 提供诊断与预测生物标志物。然而,先前基于线性机器学习方法、通过分割皮层特征来区分 MDD 患者与健康对照(HC)的尝试所报告的准确率较低。此处,我们使用了来自 ENIGMA-MDD 工作组的全球代表性数据,包含来自 30 个站点的 7,012 名参与者(N=2,772 MDD 和 N=4,240 HC),从而可进行具有可推广结果的综合分析。基于顶点级皮层特征整合可提升分类性能的假设,我们评估了 DenseNet 与支持向量机(SVM)的分类表现,预期前者优于后者。我们发现,在未见站点上估计时,两种分类器均接近随机水平(平衡准确率 DenseNet:51%;SVM:53%)。当交叉验证折包含来自所有站点的受试者时,分类性能略高(平衡准确率 DenseNet:58%;SVM:55%),表明存在站点效应。总之,顶点级形态测量特征的整合与非线性分类器的使用并未导致 MDD 与 HC 的可区分性。我们的结果支持如下观点:基于此类特征与分类器的组合进行 MDD 分类是不可行的。或许对多模态信息更精细的整合可在此诊断任务中带来更高性能。
